Our Policies

Patient Care

We provide surgical, non-surgical treatment, and retreatments for adult patients starting from 14 years and older.

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Treatment can be done the same day as the consultation. 

Please assist us by providing the following information at the time of your consultation:

• Your referral slip and a copy of your CT if applicable.

• A list of medications you are presently taking.

• If you require pre-medication prior to any dental treatment, please inform our office upon scheduling your appointment. 

• Patients under age of 18 need to be accompanied by a parent or guardian.

Appointment Cancelation Policy

We understand that scheduling conflicts occasionally occur and would appreciate a minimum 48-hour notice if cancellation of your appointment is necessary. Repeat no shows or cancelation may be subject to our office being unable to provide you with future care.

Office Financial Policy

Insurance Information: 

To minimize your first office visit, please provide us with your insurance information when you call to make an appointment. This will insure us with an opportunity to verify coverage and notify you of your portion of your out of pocket co-payment. If any questions, please call the office to verify that we are a participating provider with your insurance company.

Co-payments and deductibles are due at time of service(s) rendered for PPO patients.

Patients with no insurance: 

Full payment is due at the time of service.

For your convenience we provide a number of payment options that may be used individually or combined according to your wishes.
